Use this form if you want to request an internal review the decision on your FEG claim and cannot use FEG Online Services. You must make your request within 28 days of the decision date.

This form may only be used by Aboriginal and Torres Strait Islander people who wish to lodge a FEG claim but are unable to providing any of the Mandatory Documents specified in the FEG claim form.
